The Institute has developed safety protocols for programs. COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in our programs could increase the risk of contracting COVID-19. The Institute in no way warrants that COVID-19 infection will not occur through participation in our programs or accessing Institute facilities. Please sign below that you understand the above and are willing to assume said risks.

I agree to the following procedures and policies established by The Institute, listed below, as indicated by my checkmark.

I understand that any participant who has been diagnosed with COVID-19, who has COVID-19 symptoms, or who has had exposure in the last 14 days to COVID-19 can NOT attend.I will alert The Institute if any participant or anyone in my household has symptoms of COVID-19, such as fever, shortness of breath or persistent dry cough, in the 72 hours prior to the start of the program or during the session.I will alert The Institute if any participants or anyone in my household is diagnosed with COVID-19 within a 14-day time period after the program ends.I understand that the program may need to be cancelled on short notice due to government order, child or staff illness, or other emergency.I understand that if my family cannot attend for any of the reasons listed above, I will receive a full refund.I understand that if The Institute becomes aware of any other participants or staff that used facilities or supplies within a 14-day time period and have contracted COVID-19, I will be alerted.
